The health care sys­tem is sub­ject to per­ma­nent chan­ge. With the intro­duc­tion of the DRG sys­tem in 2001, it was impli­ci­tly deci­ded that health would beco­me an eco­no­mic good depen­dent on the for­ces of the mar­ket eco­no­my. Pri­ce com­pe­ti­ti­on, pri­va­tiz­a­ti­on, spe­cia­liz­a­ti­on, out­pa­ti­ent tre­at­ment etc. are the logi­cal con­se­quen­ces of this.


For health­ca­re com­pa­nies, this mar­ked the dawn of a new era. The trans­for­ma­ti­on from “aut­ho­ri­ties” with admi­nis­tra­ti­ve struc­tures and no respon­si­bi­li­ty for results to com­pe­ti­tively suc­cess­ful, pro­fit-ori­en­ted com­pa­nies have sin­ce been the task of tho­se invol­ved. At the same time, the sta­te is inter­vening. Pri­va­te — pure­ly pro­fit-ori­en­ted — pro­vi­ders have taken over more and more hos­pi­tals, while smal­ler hos­pi­tals have incre­a­singly had to shut their doors or be con­ver­ted into ambu­la­to­ry struc­tures. In 2018, poli­tics then incre­a­sed the pres­su­re again to achie­ve the open­ly sta­ted goal of “cold struc­tu­ral chan­ge — redu­cing ‘over­ca­pa­ci­ty’ of hos­pi­tal beds.”


The abo­ve-men­tio­ned opti­miz­a­ti­on mea­su­res are no lon­ger enough — even for lar­ge hos­pi­tals. The hos­pi­tal enter­pri­se must behave in a high­ly pro­fes­sio­nal man­ner in all are­as to sur­vi­ve the com­pe­ti­ti­on.
